Superbill Processing & Submission Fee Policy

At Balanced Living Occupational Therapy, PLLC, we are a cash-based practice and do not bill insurance directly. However, we understand that some clients may wish to seek out-of-network reimbursement from their insurance provider.

To assist with this process, we offer the following administrative services:

Superbill Preparation Only – $25 per claim

• Includes a professionally prepared Superbill with proper service codes for client-led insurance submission.

• You are responsible for submitting the claim to your insurance provider.

Superbill Preparation & Submission Assistance – $50 per claim

• Includes the creation and submission of your Superbill on your behalf to your insurance provider.

• Includes assistance with verifying correct submission and guidance on next steps.

• We do not guarantee reimbursement, as coverage depends on your individual insurance plan.

Clients are welcome to request a Superbill at any time and submit it on their own without incurring additional fees.

Please note: This is an administrative service and does not influence whether or not your insurance will reimburse you.
