Services Balanced Living OT, PLLC provides occupational therapy services on a private-pay basis. Details of our full services are available on our website. All services are delivered in accordance with professional standards and applicable laws. Occupational therapy services provided by Balanced Living OT, PLLC do not include medical diagnoses or treatments outside the scope of occupational therapy practice. For medical concerns, please consult with a licensed medical professional. Services include in-home visits and virtual sessions, depending on your preferences and needs.

Payment Payment is due at the time of service unless otherwise agreed upon in writing. Pricing is based on factors such as location, service type, and complexity of the case. These details will be discussed and agreed upon during the scheduling process. We do not bill insurance directly. Clients are responsible for seeking reimbursement through their insurance provider using the superbill provided. Late or missed payments may result in the suspension of services.

Required Documentation All required documentation, including intake forms and consents, must be completed and submitted prior to the scheduled session. Failure to provide these documents may result in delays or rescheduling of services.

Cancellations and Refunds Appointments canceled with less than 24 hours’ notice may be subject to a cancellation fee. Refunds are not available for completed services. If Balanced Living OT, PLLC is late to an in-home appointment due to unforeseen circumstances, the session will still be completed in full, and the cost remains the same. If Balanced Living OT, PLLC needs to reschedule due to unexpected events, we will notify you as soon as possible to coordinate a new appointment time. No refunds will be issued for digital products - we truly believe in giving more than receiving, and each of our products and services is designed by keeping this core principle in mind.
